In consideration of the sum of $________________ this contract and guarantee is made this________day of ______________________, 20___ between Jewels Puppyland, (seller) and________________________________ (buyer). Seller agrees to sellto buyer a female/male dog whose color is __________________ (herein referred to as puppy). The puppy was born on _____________________________, 20_______ and sired by_____________________________________________ registration organization and number____________________________. The dam is ______________________________________registration organization and number ____________________________________________.The puppy is registerable with the ______________________ and appropriate forms have been furnished to buyer.

It is the sole responsibility of the buyer to carry out the process required and pay any expenseinvolved with the registration procedure using the forms provided to the buyer.The seller guarantees that the puppy is in good health at the time of sale and has had at least 1 (usually 2sets) set of age appropriate vaccinations and de-wormings as well as a thorough physical examination by a licensed veterinarian. The seller guarantees the health of the puppy for 3 days until the examination of the puppy by a licensed veterinarian chosen by and paid for by the buyer. If during the course of that examination, the puppy is found to be in unsatisfactory health and written documentation is provided to the seller by the buyer's licensed veterinarian, then seller shall, upon return of the puppy and the registration papers, refund the full purchase price of the puppy to the buyer. Excluded from this health guarantee are worms and other parasites as they are common to puppies and dogs and should pose no lasting ill effects to the puppy if treated promptly. Seller has taken every precaution to avoid the presence of worms or parasites in the puppy. The seller further guarantees that the puppy is free from any crippling or life-threatening hereditary or congenital defects for a period of 12 months from the time of sale. Proof of any defect must be provided, in writing, from a licensed veterinarian and the puppy and the puppy's registration papers must be returned to the breeder. If a replacement puppy of similar value is available, one may be provided. If a similar puppy is not available, the breeder will refund the original purchase price of the puppy. This guarantee will be void if the puppy is destroyed before the breeder is notified of the condition. All veterinary expenses for diagnostic purposes are the sole responsibility of the buyer. Spaying/neutering is recommended for this puppy but is not required. Spaying or neutering ayoung dog may reduce the risk of cancer in later life and will usually make for a nicer pet. The buyer's veterinarian will be able to provide more information on the benefits of spaying or neutering the puppy.

Jewels Puppyland cannot be held responsible for any expenses incurred from shipping or veterinarian care.

Please be aware that we estimate the size and color of your puppy to the best of our ability. Sizing is based on knowledge of our dogs, their pedigrees and the individual puppy's growth. However a puppy's environment and diet can have an affect on size, color, personality etc. We cannot control their environment, therefore cannot guarantee size, color or personality.

Changes such as travel, diet etc. can result in stress for your puppy. Stress related problems include hypoglycemia and coccidiosis. These stress related problems are not covered by our guarantee and Miss Majestic Kennel cannot be held responsible

It is the sole responsibility of the buyer to obtain veterinary care for the puppy once the sale hasbeen completed and to keep the puppy current on all vaccinations and parasite checks inaccordance with the American Veterinary Medical Association. This includes any emergencycare as well as routine examinations, vaccinations, and heartworm treatment as well as any othertreatments which may be recommended by the buyer's veterinarian.The seller accepts no responsibility for the illness or death of any puppy due to the buyer'sinability or negligence in providing proper veterinary care for the puppy. No other warranties or guarantees, expressed or implied, are made in this contract.
